SCOPE OF SERVICES PROVIDED FOR Health Path

The service is provided to employees of named companies who have a contractual agreement with Centric Health. The employee type and eligibility conditions are governed by the company engaging Centric Health. https://www.centrichealth.ie/.

To enter the program employees contacts Centric Health @ healthpath@centrichealth.ie or Centric Health to contact the individual employee to arrange a suitable appointment time.

Centric Health use AMS platform (medapp) to carry out online booking, record personal data, collect and record the test data and issue the report with all test results and our recommendation to the individual employee. https://advancedmedicalservices.ie/

Consultations, as described below, are paid for by the employer.

An SMS or e-mail message will be issued to you if there are any delays in the service. Appointment confirmations will be issued by an SMS or email message. Both forms of communication are supplied by employees.

An employee will receive and initial online questionnaire.

The employee attends the Health Path program in Clifton Court Medical Centric Health for the following purpose:

Appointment one – the employee will meet with a Centric Health nurse and complete an extensive medical questionnaire.

Appointment two – the employee will meet with a Centric Health Doctor and an in-depth discussion will take place regarding health markers. This will occur one or two weeks after appointment one. This decision is made by the employee.

A complete and comprehensive report is issued directly to the employee a week later.

Centric Health will make a courtesy call circa six weeks after the second consultation to check back in with the employee to see how the participant is getting on

A follow-up appointment with the nurse for repeat testing circa twelve weeks after first doctor consultation

A follow-up appointment with a doctor approximately one or two weeks later.

Follow-up report issued one week after doctor appointment. During this consultation:

1. Option for interventions to be made available to employees at any stage of the program
2. All charges outside the service schedule are paid for by the individual employee.
